Willamette View offers employees an opportunity to work in an enviroment that is focused on our resident's overall experience, providing nursing care and assistance that supports our mission to allow "The art of Living Well" for each resident.  This position will be a Full Time position working the day shift in our Assisted Living areas of our Health Center.

JOB SUMMARY: Responsible for managing the care plan for residents within assigned area.  Responsible for assuring appropriate follow through of care directions by all members of the assigned nursing team therefore assuring high quality of services in a manner that demonstrates a good understanding of the principles and values of Assisted Living and care of persons with dementia.  In addition, that which complies with state laws and regulations and emphasizes shared responsibility with the resident, and supports the resident's choice, dignity, independence, individuality, privacy and right to decision making.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

1.       Maintains working knowledge of resident's medical diagnosis and current medical status and uses this knowledge in      assessment of the resident's physical, emotional, social, spiritual and intellectual needs.

2.       Legally documents resident information using appropriate terminology, spelling and abbreviations according to policies of facility and state and federal regulatory agencies.

3.       Demonstrates an expertise in the execution or supervision of skilled nursing procedures; i.e. medication and treatments within the scope of licensure.

4.       Makes daily rounds and identifies early signs and symptoms, changes in resident's physical and psychological status and consults with supervisor, physician, NM or DHS as necessary.

  1. Has working knowledge of and follows individual medical/nursing plan of care.
  2. Coordinates resident services with other departments for overall efficiency and well-being of the resident.
  3. Modifies/updates Service Plan as a resident's needs change or decision changes are made.
  4. Supervises practice of Licensed Practical Nurses.
  5. Trains, monitors and supervises staff in ADL performance and accurate documentation of resident services.
  6. Assures that medications are administered accurately; review medication records and resident responses.
  7. Interacts appropriately with families, pharmacists, physicians, office staff and others to ensure that residents needs are met; promotes good facility public relations during these interactions.
  8. Keeps Administrator, DNS/RSC, RSM, RCM and other pertinent staff informed of resident service issues and problems.

15.      Is aware of staffing needs, and has knowledge of CNA's strengths and weaknesses and advocates for consistent  personalized assignments/care.

16.      Attends in-service education within facility and outside workshops to obtain at least 15 hours CEU's per year. 

17.      Assures that new admissions have preliminary assessment and care plan on chart within 8 hours of admission.

18.      Shows empathy, compassion, and genuine concern for the well-being of all residents and families and communicates in professional way as situation requires.

19.     Is current on legal implications of providing care ethically and according to nursing standards and nurse practice act.

20.     Recognizes the advantages of collaborative relationship with peers and uses this to increase his/her knowledge.

21.     Collaborates with other departments and reports persistent unresolved problems to NM and/or DHS.

22.     Is aware of safety standards and follows all established safety rules.  Detects and reports safety hazards to appropriate department for resolution.

23.     Maintains strict confidentiality with staff and resident issues.

24.     Is able to prioritize a multi-demanding role demonstrating initiative and appropriate judgment.

25.     Is flexible and dependable for staffing patterns to meet residents needs.

26.     Reports to work on time and as scheduled.  Follows departmental reporting policies for absences.  Completes daily duties as scheduled and completes non-scheduled duties as time allows.

Maintains appropriate uniform and maintains professional appearance in compliance with policies.

 NON-ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

 May serve as a representative for department on the safety committee or other ad hoc committees.

 May perform duties of other staff in department as need arises.

QUALIFICATIONS AND ABILITIES TO PERFORM JOB SUCCESSFULLY: Registered nurse graduate of a state approved school of nursing with successful completion of state board nursing exam and current unencumbered license.  Maintains current certification in CPR. Demonstrates leadership ability and desire for professional growth.  Demonstrates knowledge and competence in practicing the principles of good nursing care.  Able to interact well with staff as well as residents and their families.  Able to prioritize multi-demanding role using initiative and judgment. 

SAFETY HAZARDS: Possible exposure to blood or body fluids.  Requires considerable bending, twisting, reaching and lifting.   Exposure to slippery floor surfaces.

Hiring wages start at $25.32 up to midpoint of $31.65 per hour depending on experience; $.75 eves, $2.00 night differential

 

EQUIPMENT USED: Wheelchairs, walkers, electric beds, hydraulic lift, medication carts and computer.
